What Is Co-Management?
Co-Management describes a model of care in which pediatric sub-specialists lend expertise to help primary care providers (PCPs) gain core competencies necessary to provide appropriate levels of care, in a Medical Home environment, for children with higher volume, lower acuity, or other appropriate conditions. Co-Management Plans for appropriate conditions are continually developed and include a central algorithm, intial and follow-up visit templates, and handouts for PCPs and patients/families.
Providers interested in utilizing available Co-Management Plans are encouraged to view the accompanying Continuing Medical Education (CME) workshops for Co-Management. These presentations combine a didactic overview of each condition presented by the sub-specialist co-author, with a walk-through of a case study using the tool-kit materials, presented by the PCP co-author.
