 The pilot study of Co-Management as a model of care between primary care providers and pediatric sub-specialists is complete. We will be posting the results of this study very soon.




We are currently in the process of updating this site to offer comprehensive tools to assist you in co-managing a variety of pediatric conditions. Please check back soon to access these resources. What Is Co-Management?

The Co-Management model of care is characterized by:

  • Pediatric sub-specialists lending expertise to help primary care providers gain core competencies necessary to provide appropriate levels of care in a medical home environment for children with higher volume, lower acuity conditions.
  • Improved collaboration and communication between primary care providers and pediatric sub-specialists.
  • A "best of both worlds" situation for patients who are able to access the expert knowledge of pediatric sub-specialists while receiving more of their care from their primary care provider, increasing their comfort and making treatment more convenient.
